29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) 5200 S Quebec St, Right of Way, Greenwood Village, CO 80111: On or about June 21, 2023, and at times prior, QP Services, LLC employees were exposed to struck by hazards while attempting to repair a water leak in an unprotected trench. Rock was within two feet of the south and east edges of the trench. The rock was part of the retaining wall that was removed on the east side of the trench. The employer failed to ensure excavated materials were at least two feet away from the edge of the excavation to prevent materials from falling or rolling into the excavation.
