29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) On or about September 6, 2023 - Assembly, employees were exposed to struck-by and caught-in hazards when the employer did not ensure that employees affixed lockout device(s) on equipment such as, but not limited to, the biscuit cutter, bag sealer, and 'doughboy' before employees cleared a jam and performed clean-up and sanitation at the equipment.
