29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about August 8, 2023, an employee in a 5.2 foot deep excavation was exposed a to a cave-in hazard without a protective system when completing a storm sewer tie-in located at 6754 Stow Road Hudson, OH 44224. Karvo Companies, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1556035, citation number 1, item number 3 and was affirmed as a final order on 12/06/2021, with respect to a workplace located at Windfall at SR 18 Medina, Ohio 44256.
