29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in 29 CFR 1910.147(d)(1)-(6) and was not done in sequence as required by 29 CFR 1910.147(d). a). New Horizons Baking Co. On or about July 28, 2023, at the location of 211 Woodlawn Ave. Norwalk, OH: The employer failed to ensure established procedures for the application of energy control was done in sequence to ensure the LeMatic Inc. BF-25W Band Slicer is fully deenergized prior to the servicing/maintenance task. The employees were replacing the Band Slicer blade without the benefit of fully controlling all potentially hazardous energy. Due to this condition, the employee was exposed to a caught in hazard.
