29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. On or about July 24, 2023, employees of a maintenance shop were exposed to a struck-by and crush -by hazards at a worksite located at 1900 E Blaine St, Springfield MO. A maintenance technician was allowed to independently conduct maintenance and repair tasks without having completed authorized employee training in recognition, isolation, and control of residual and hazardous energy and the procedures for locking out all sources of energy including but not limited to pneumatic, mechanical and electrical energy.
