29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Each employee was not required to: (1) prepare for shutdown, (2) turn off and shut down the machines, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric and pneumatic energy sources, (4) affix lockout or tagout devices to each energy-isolating device, (5) ensure that all potentially hazardous stored or residual energy is relieved, disconnected, restrained, and otherwise rendered safe, (6) verify the effectiveness of energy isolation. a) On or about June 15, 2023, the employer did not ensure that (d)(1) through (d)(6) was followed by authorized employees performing maintenance work on the Disamatic D3 by properly securing the electric, pneumatic, water, hydraulic and thermal supplies prior to starting work. Employees were exposed to caught-in hazards associated with the movement of the drive arm while lining up the bearings. b) On August 30, 2023, the employer did not ensure that (d)(1) through (d)(6) was followed by molding employees performing cleaning tasks on the Disamatic D3 by properly securing the electric, pneumatic, water, hydraulic and thermal supplies prior to starting work. Employees are exposed to caught-in hazards associated with cleaning out spilled hydraulic fluid at the cylinders located in the mold pour area.
