29 CFR 1910.23(b)(13): The employer did not ensure that no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder: On or about June 5, 2023, employees were exposed to fall hazards up to four feet when climbing a Little Giant Safety Step Model 10410BA while carrying elevator cab roofs and the employer had not ensured employees did not carry loads that could affect balance while climbing a portable ladder.
