29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a)Northland Concrete and Masonry Company, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the exposing employer did not ensure employees were protected from falls while using personal fall arrest systems that were anchored to unsecured formwork on a suspended Doka working platform. The formwork separated from the platform and employees fell to the ground below. This condition exposed employees to fall, crushing, and struck-by hazards.
