29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment. a. On or about May 22, 2023, at the Wynnewood Refinery in Wynnewood Oklahoma, employees were exposed to fire and chemical release hazards when the employer did not establish and implement written troubleshooting procedures for employees tasked to troubleshoot an automated valve. b. On or about May 23, 2023, at the Wynnewood Refinery in Wynnewood Oklahoma, employees were exposed to fire and chemical release hazards when the employer did not establish and implement written procedures for employees tasked to break open an automated valve and physically maneuver the valves internal components.
