29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. On or about May 15, 2023, at Marathon Galveston Bay Refinery located in Texas City, Texas, the employer failed to comply with recognized and generally accepted good engineering practice, such as NFPA NEC-70 when pump 303-J was not electrically interconnected (or bonded) in Ultraformer 3 unit that processes Naphtha. Exposing employees to fire, burn, and explosion hazard that resulted in fatal injuries.
