29 CFR 1910.147(c)(4)(ii): The lockout/tagout procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance. a) Production area, Stamped Fittings, Inc., 217 Lenox Ave., Elmira Heights, NY 14903, Omera Gasket Machine 1, on or about 4/26/2023. The written lockout/tagout procedures were not adequate for the Omera Gasket Machines, in that they did not clearly and specifically outline the specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. Procedures did not include the control of all energy sources, including, but not limited to, pneumatic and hydraulic sources of energy. b) Production area, Stamped Fittings, Inc., 217 Lenox Ave., Elmira Heights, NY 14903, Omera Gasket Machine 2, on or about 4/26/2023.
