29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about April 17, 2032, personal protective equipment was not issued to operators prior to engaging in maintenance and servicing activities such as, troubleshooting or inspecting the Clean-In-Place (CIP) flow plate line connections in the "Silo Alley," exposing employees to thermal burn hazards from hot water approximately 140-150 degrees Fahrenheit.
