29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about April 10, 2023, employees were exposed to machine hazards associated with thermal and chemical burns during removal of the electronic expansion valve, located on the Carrier Model 30 GN/GT Liquid Chilling Unit at the Abraham Lincoln Capital Airport. The employer failed to implement energy control application steps as the authorized employee had not affixed a lockout or tagout device to each energy isolating device used to control refrigerant line pressure prior to performance of servicing work [per the 1910.147(d)(4)(i) requirements]. As a result, the remaining applicable energy control elements, dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
