29 CFR 1910.119(j)(4)(iii):The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience. On or about April 5, 2023, employees were exposed to anhydrous ammonia release hazards when the employer did not ensure dual relief valves on Compressor #108 in the Cook Refrigeration Plant were replaced and/or recertified according to their employed RAGAGEP including but not limited to IIAR Bulletin 110 (Start Up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems)Section 6.6.3 Revision, May 24, 2007.
