29 CFR 1910.146(c)(9)(i): Each contractor who was retained to perform permit space entry operations did not obtain any available information regarding permit space hazards and entry operations from the host employer: On or about March 13, 2023, at the tunnel of Silo #1 and Silo #2, employees entered the tunnel under the silos that contained atmospheric hazards such as but not limited to phosphene, nitrogen dioxide, and carbon monoxide gases to extend a conveyor. The employer did not obtain from the host available information regarding the permit space hazards.
