29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about March 28, 2023, at an excavation site located in an open field on the corner or NW 63rd and Grand in Nichols Hills, OK, two employees were observed on their knees, leaning and reaching into a trench to hold to top of the ladder as employees used the ladder to exit the trench that was 15'7" in depth.
