29 CFR 1918.24(i)(1)(i): Ladders with broken, split, or missing rungs, cleats, or steps were used or were not tagged as unusable or removed from the vessel: (a) Follansbee Cleaning Fleet, Vessel MEM 2442, 54 Landry Lane, Follansbee, WV: On March 22, 2023, during a barge cleanout, an employee was exposed to an approximately 15 foot fall while utilizing a ladder to access that barge that had a missing rung.
