29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. a) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 03/15/2023, the employer exposed employees to inhalation and skin contact hazards associated with a catastrophic release of pressurized liquid anhydrous ammonia, in that, a pressure gauge and its 1/4 inch service/gauge valve on a 25,000 lb. approximate ammonia suction accumulator vessel V-33 was inoperable and not able to be read. Line breaking activities were commencing at/near the gauge location, and contractors involved were unable to get an accurate read of the pressure inside the pipe where line breaking was going to commence.
