29 CFR 1910.178(l)(2)(iii): Operators training and evaluation was not conducted by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence: On or about March 3, 2023, employees were being trained and evaluated by the safety manager who did not have a current certification or train the trainer training prior to training, evaluating, and certifying industrial truck operators, exposing employees to struck by and caught in or between hazards.
