Location: a) On the second floor of the jobsite at 126 Lafayette Street, New York, NY 10013: On or about March 7, 2023, Employer did not instruct employees in the recognition and avoidance of unsafe conditions such as a masonry wall used as a retaining wall against piled debris and was incapable of safely supporting the imposed load, and exposure to respirable crystalline silica during demolition operations.
