29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: (a) On or about March 2, 2023, and at times thereafter, the employer had not conducted annual periodic inspections of the energy control procedures for track/tunnel equipment. Employees were exposed to caught-in and struck-by hazards associated with unexpected machine start-up or release of electric and pneumatic energy sources during servicing and maintenance tasks due to the lack of periodic inspections to identify any deviations or inadequacies for correction.
