29 CFR 1910.146(d)(3)(iv): Under the permit space program, the employer does not develop and implement the means, procedures, and practices necessary for safe permit entry operations, including, but not limited to, purging, inerting, flushing or ventilation the permit spaces as necessary to eliminate or control atmospheric hazards: On or about March 2, 2023, the employer did not ensure that atmospheric hazards were eliminated and/or controlled while an employee was working inside #4029 Mississippi Tank Company manufactured tanker trailer. An employee who entered the trailer to conduct an inspection was exposed to atmospheric hazards, such as oxygen deficiency and flammable gasses. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
