29 CFR 1910.23(d)(4): The employer did not ensure that the side rails of through or sidestep ladders extend at least 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder: a) General Warehouse Area - Employees required to access the building roof were exposed to a fall hazard of approximately eight (8) feet in that the ladder used to access the roof was not installed such that the side rails extended at least three (3) feet above the upper landing being accessed, on or about March 1, 2023.
