On a 6:12 pitch roof of a commercial business, at a temporary worksite located at 5556 Fort Caroline Road, in Jacksonville, FL; On or about January 25, 2023, the employer exposed employees to fall hazards, ranging from 6 to 18.5-feet, in that fall protection was not utilized by the employees while performing roofing work.
