29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years: a) Pasco, Inc. - Workplace located at 7250 Paschall Avenue Philadelphia, PA 19142 - Baling Machine Area: On or about January 26, 2023, the employer did not ensure an evaluation of each powered industrial truck (PIT) operator's performance that utilized the Nissan Diesel Forklift 167301 to move bales of scrap metal and other materials was conducted at least once every three years, exposing employees to struck-by hazards.
