29 CFR 1910.23(d)(4): The employer did not ensure that the side rails of through or sidestep ladders extend at least 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder. a) Saw Room; on or about January 6, 2023 and at times prior to, maintenance employees were exposed to falls hazards, in that the employer did not ensure that the fixed ladder attached to the Barry Brown circular saw had rails extending at least 42 inches above the saw platform to prevent falls from a 15 ft. elevation onto the concrete floor. b) Saw Room; on or about January 6, 2023 and at times prior to, maintenance employees were exposed to falls hazards, in that the employer did not ensure that the fixed ladder attached to the Brown saw had rails extending at least 42 inches above the saw platform to prevent falls from a 15 ft. elevation onto the concrete floor.
