29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Jobsite: 1901 E York St, Philadelphia, PA 19125 - On or about January 10, 2023, the employer did not provide a safe means of access to and from each level of a supported tubular welded frame scaffold, which was erected within the elevator shaft of a 6 story residential construction building, by the use of portable ladders, hook-on ladders or other acceptable means. Employees climbed the scaffold end frames on the 5th level of the building to access the 6th level of the building, which measured 7 feet above the walking working surface of the 5th level, and approximately 60 feet above the ground level through the elevator shaft opening.
