29 CFR 1910.119(e)(5): The employer did not establish a system to communicate the actions to be taken, as a result of the process hazard analysis team's recommendations, to operating, maintenance and other employees whose work assignments are in the process and who could be affected by the recommendations actions: On or about December 13, 2022, and at times prior thereto, the employer failed to communicate the actions of the process hazard analysis team's recommendations to the operating, maintenance and other employees whose work assignments are in the process and who could be affected by the recommendations actions after the conduct of the process hazards analysis revaluations. Employees were potentially exposed to a release of ammonia which could result in serious injury or death.
