On or about November 08, 2022, where there was a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or possibility of accumulation no longer existed as required for Tank 48055 in the OP-2 area and exposed employees to caustic burn hazards.
