29 CFR 1910.146(d)(4)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not ensure that testing and monitoring equipment needed to comply with 29 CFR 1910.146(d)(5) was maintained properly. a. On or November 28, 2022 and at times prior, the employer did not ensure that the Gas Clip Technologies Multi Gas meter, Model: MGC-Simple, SN: D4SM285121, used by employees to test tanks prior to and during confined space entry for tank cleaning operations was maintained in a safe operating condition by regularly bump testing the meter to ensure accurate readings of the space.
