29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. a) Ziegler Blvd/West of David Langan Drive W - Mobile, AL (Maintenance Hole): On or about November 15, 2022, the employer exposed employees to fall hazards in that fall protection such as but not limited to a cover was not utilized by workers/employees performing construction activities to raise an open manhole to grade level.
