29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. Production Area, Donut DCA and The Depositor - On or about October 27, 2022, while performing production change-over that included, but were not limited to, attempting to verify that the Donut DCA and the Depositor Head did not have the energy effectively isolated, no verification of Lockout/Tagout operations was performed on the Donut DCA Head or the Depositor to confirm that the equipment was effectively isolated and rendered safe, which created struck-by hazards.
