29 CFR 1926.651(c)(2): A means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress was not located in trench excavations more than 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: On or about October 26, 2022, at the jobsite, employee #1 was working inside of the excavation approximately 570 feet long were not provided with means of egress every 25 feet, exposing employees to the hazard cave-ins in the event of a collapse.
