29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee was protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. On or about 10/5/2022, the employer did not ensure that employees walking in the aisleways during machine cleaning and sanitation were protected from stepping into uncovered floor drains that exposed employees to trip and fall hazards. The floor drain covers that were initially lifted off for removal of scrap meat were not replaced in a timely manner once this task was completed.
