29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Discharge Conveyor on the Milling Machine - On or prior to October 5, 2022, an authorized employee for D. A. Collins Construction Co., Inc. did not receive adequate training in lockout/tagout for the control of the hazardous energy for the Wirtgen W220i Milling Machine ((#6485)(Serial Number: 07220018)) when performing servicing and/or maintenance tasks such as, but not limited to, inspecting the Discharge Conveyor while the Discharge Conveyor is running at a low speed, and without guards in place exposing employees to caught-in hazards.
