29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Repro/Engineered Resin Department - On or about 10/27/22, employees were using an ALTA 215 Super (serial number 4907301080) fork lift truck to unload plastic chips from tractor trailers and to load the chips into the chips bin. The headlights of the fork truck were broken and did not light, the tires were in need of replacement and the back panel was held on with a ratchet strap. The PIT was unsafe and was not removed from service.
