29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a) On or about September 16, 2022, employees were exposed to tripping hazards due to trench drains, which were approximately 12 inches wide, 14 inches deep, and 4 feet long, missing covers.
