29 CFR 1910.23(d)(4): The employer did not ensure that the side rails of through or sidestep ladders extend at least 42 inches (1.1 m) above the top of the access level or landing platform served by the ladder. a. Blanking Press Pit - On or about September 27, 2022, the fixed ladder on the north side of the pit, at the end of the press table, did not have side rails that extended at least 42 inches above the landing, exposing employees to a 15 to 20 foot fall hazard onto concrete.
