29 CFR 1910.146(c)(1):The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a) DOA Line 1 Tower; On or about August 24, 2022 and at times prior, employees were exposed to entrapment and struck-by hazards, in that employees were required to enter the tower on DOA line 1 to perform cleaning without the employer performing an evaluation to determine if any hazards were present. b) Oven on Line 1; On or about August 24, 2022 and at times prior, employees were exposed to entrapment, respiratory, and burn hazards, in that employees were required to enter the ovens to perform cleaning without the employer performing an evaluation to determine if any hazards were present. c) Oven on Line 2; On or about August 24, 2022 and at times prior, employees were exposed to entrapment, respiratory, and burn hazards, in that employees were required to enter the ovens to per perform cleaning without the employer performing an evaluation to determine if any hazards were present. d) Dust House; On or about August 24, 2022, and at times prior, employees were exposed to entrapment and struck-by hazards, in that employees were required to enter the dust house to perform cleaning without the employer performing an evaluation to determine if any hazards were present.
