29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): (a) On or about 7/26/2022, 3100 9th Ave N, Birmingham, AL, on the 4:12 pitch roof, employees using pole saws to cut trees were not protected from 27 foot fall hazard due to unprotected skylights.
