When an employer arranged to have employees of another employer (contractor) perform work that involved permit space entry, the host employer did not inform the contractor that the workplace contained permit spaces and that permit space entry is allowed only through compliance with a permit space program meeting the requirements of this section. (a) On 8/15/2022 and at times earlier, in the Coke Hopper, contractor employees were exposed to falls of up to 13 feet to the bottom of the hopper, and hazardous energy at the point of operation for the vibrator because ACICPCO did not inform CORE that work in the coke hopper involved entry into a permit space and that entry into the hopper was allowed only through compliance with a permit space program meeting the requirements of this section.
