29 CFR 1910.146(d)(4)(iv): The employer did not provide, maintain or ensure employee use of personal protective equipment insofar as feasible engineering and work practice controls did not adequately protect employees (a)On 8/15/2022 and at times earlier, in the Coke Hopper, employees were exposed to falls of up to 13 feet to the bottom of the hopper, and hazardous energy at the point of operation for the vibrator because the employer did not personal protective equipment, such as fall protection equipment, requiring employees to lean on the inwardly converging walls.
