29 CFR 1926.651(c)(2):A means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress was not located in trench excavations more than 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. The employer is failing to protect employees from fall and cave-in hazards. This was most recently documented on August 9, 2022, at the construction site located at the Southwest corner of S 2nd St and S Metcalf Road, Louisburg, Kansas whereby no ladder or a safe means of egress was provided while employee were in the trench. The trench measured approximately a depth of over 6 feet, a width of 3 feet, and a length of over 20 feet.
