29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the areas protected by shields: (a)H & W Contracting, L.L.C. at 217 East 6th Avenue, Redfield, SD 57469: On or about and at times prior to August 11, 2022, the employer did not ensure that employees were protected from the hazard of cave-ins when entering or exiting the areas of a trench protected by shields. This condition exposed employees to cave-in and struck-by hazards associated with excavation wall collapse.
