On or about July 24, 2022, near the evisceration line A-Frame ladders are used by employees to clean the evisceration line including overhead parts on wet floors. a) a 10-foot ladder was used on a wet floor without being secured from slipping. b) a 6-foot ladder was used on a wet floor without being secured from slipping.
