29 CFR 1910.119(g)(2): Refresher training was not provided at least every three years, and more often if necessary, to each employee involved in operating a process to assure that the employee understood and adhered to the current operating procedures of the process. At a worksite located at 200 N Banquet, Marshall, MO 65340: Employees were exposed to toxic, cryogenic, and fire hazards in the event of an anhydrous ammonia release. This was last demonstrated on or about July 18, 2022, and at times prior to, when the employer had not provided refresher training at least every three years on the written operating procedures for line-breaking operations. Employees were injured when a release of anhydrous ammonia occurred causing burns and inhalation injuries.
