29 CFR 1926.651(f):A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation, and the operator does not have a clear and direct view of the edge of the excavation. On or about July 1, 2022, at the Montecillo Unit 3 excavation jobsite, an excavator was operated adjacent to an excavation was required to approach the edge of the excavation, the operator did not have a clear view of the edge of the excavation, and the employer did not provide a warning system such as barricades, hand or mechanical signals, or stop logs.
