Location: a) On the first floor of the jobsite: On or about June 14, 2022, an employee was removing an existing wooden structural beam on the first floor of the jobsite when the beam collapsed. The employee fell twelve feet onto the basement level. Employer did not train the employee in recognition and avoidance of the fall hazard from demolition.
