29 CFR 1926.959(d)(2): A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provide timely warnings such that the minimum approach distance was being maintained before the minimum approach distance required by paragraph (d)(1) of this section was reached. On or about June 15, 2022, at an electrical sub-station, there was no designated employee to observe and provide timely warnings to an employee in an aerial lift who exceeded the minimum approach distance to energized lines and was exposed to electrical hazards.
