29 CFR 1910.67(c)(2)(vii): Outriggers were not used and positioned on pads or a solid surface and the wheel chocks were not used when using an aerial lift on in incline. a) Worksite, 61 Paddock Farm Rd. Brewster, NY: On or about June 15, 2022, an employee was trimming a pine hedge row from the bucket of an Altec LR7 Front Mount Aerial truck with the boom extended out on the driver side of the truck approximately 40 feet above the ground. The aerial truck was set on an incline with the outrigger shoes set on outrigger pads. The driver side outrigger shoe was not properly positioned on the outrigger pad and slid off the pad and into the soft ground causing the aerial truck to tip over. b) Worksite, 61 Paddock Farm Rd. Brewster, NY: On or about June 15, 2022, an employee was trimming a pine hedge row from the bucket of an Altec LR7 Front Mount Aerial truck approximately 40 feet above the ground. The aerial truck was set on an incline with only one wheel chock placed under the rear driver side tire.
